Sick (fit) notes

To Request a new Sick (Fit) Note, you will need to self certify on the Gov.uk Website for the first 7 days of your sickness.

When you have completed your self-ceertification, please book a review with the doctor for your first sicknote.

If you require an extension of your sicknote, you can call the surgery on 01509 261843 and we will send you a questionnaire to complete. When this is returned we will send your questionnaire to the doctor for review.

When the extension has been issued, this will be sent to your mobile phones.

If the doctor requires you to have a further review of your symptoms, we will let you know to book an appointment.
