Patient Information Leaflets

Self-certification SC2 sicknote.

If employees are off work for 7 days or less, they do not need to give their employer a fit note or other proof of sickness from a healthcare professional.

When they return to work, their employer can ask them to confirm they’ve been off sick. This is called ‘self-certification’. The employer and employee will agree on how the employee should do this. They might need to fill in a form or send details of their sick leave by email.

When Should I worry?

Having an ill child can be a very scary experience for parents. If you understand more about the illness it can help you to feel more in control. This booklet is for parents (and older children) and deals with common infections in children who are normally healthy. It is not meant for children who have ongoing health problems such as asthma, heart, or kidney problems. You should seek medical help for children who are less than 6 months old. Babies younger than this can respond differently to infections.

The Bridge Housing

All of our services meet specific needs and eligibility is determined by age, location and circumstance. The survey on their website is designed to take you through a series of questions one by one, with the aim of identifying the most suitable service for your current situation. Once complete, you can go on to find our more information about the service, or make a self-referral. Once you have made a referral, one of our staff will be in contact with you to progress your application.
