New Patients

You are welcome to register with Bridge Street Medical Practice if you live within the Loughborough area.

We have also taken on a new Enhanced Service with NHS England to provide registration for patients who live outside of our area, however this does come with certain criteria. For example, the practice would not be obliged to visit due to the out of area address.

In order to register you at the practice, we require you to complete the relevant registration form(s). These forms are available from reception and can be downloaded online (please see below).

Please return the completed form, WITH your NHS number if you have been registered with the NHS before. Forms will need to be handed in to reception. We will need to see photo ID along with a letter dated within the last 14 days. This will need to include your name and current address on it. The registration form will then be checked by a receptionist to make sure we have all the correct information to be able to register you.

For children under the age of 16, please ensure you bring a copy of your Child’s Immunisation Record (known as the “RED BOOK”)

Please note: We may not be able to accept this registration form without it.

Acceptable forms of Photo ID:

• Passport.
• Driving Licence.

Acceptable forms of Proof of address (must be dated in the last 4 weeks)

• Utility Bill.
• Bank Statement.
• Rent Agreement Letter.
• Solicitors Letter.

Note: We may also accept medical cards as a form of ID, these must have an up to date address on it.

Please print and complete the following forms and return to reception along with a copy of your Child’s immunisation record:

Patients aged 16+:

Please print and complete the following (2) forms and return to reception. Please ensure your NHS number is on the form (you can obtain this from your previous practice, if you have been registered with the NHS before). If you have not been registered with the NHS before, please provide us with a photocopy of two forms of ID (Photo ID and Proof of Address):
